If you are a medical professional, treatment provider, or family member seeking to refer a client to Mainspring Recovery, we’re here to help make the process as smooth and effective as possible. We offer a pre-screen process, professional referral support, and specialized programs for justice-involved individuals.
Submit a Referral Pre-Screen
Please complete the following referral pre-screen form or contact our Admissions Department directly.
Required Information for Referral:
- Referring Provider Name and Organization
- Contact Information (phone & email)
- Client’s Full Name, Date of Birth, and Contact Info
- Relevant Medical and Substance Use History
- Insurance Information (if available)
- Reason for Referral and Any Urgency Details
- Attach Supporting Documentation (Optional: Assessments, Medical Records, etc.)

Justice-Involved Referrals
Mainspring Recovery has extensive experience working with individuals referred through the legal system. We partner with public defenders, probation officers, reentry programs, and courts to provide a structured, high-quality treatment option for justice-involved clients.
To refer a justice-involved client, please contact our admissions department or email directly with details about legal status, required documentation, and any relevant deadlines or conditions. We will work with all stakeholders to ensure a smooth and compliant transition into care.
